Your formation documents (Articles of Organization or Incorporation)
Registered agent name and address (even if its you)
Your business address, phone and email as they appear on your listings
Business bank account details (name on the account and when it was opened)
Revenue figured for the last 12 months, or your projections if you are pre-revenue
Any business credit accounts or vendor accounts you already have
If you are missing something, submit anyway and note it in the form. We will follow up.
